Shoulder pain, frozen shoulder, and what an exam sorts out first

On this page

The shoulder announces itself in ordinary reaches. A seatbelt, a back pocket, a bra clasp, the mug on the second shelf. One week the reach is awkward. A month later the arm stops before the hand arrives, and the other arm has quietly taken over getting dressed. Night is its own chapter. Rolling onto that side wakes you, or the ache sits there until you prop the arm on a pillow and bargain with sleep. People call all of this "shoulder pain" and then ask whether a chiropractor, a shot, or a surgery is the category. The category is too big. An exam has to sort rotator cuff pain, frozen shoulder, and pain that is really coming from the neck before a treatment is even a coherent sentence.

What should interrupt the sorting

Some shoulders should not be eased into a course of visits. A shoulder that slipped out of joint, or that now feels as if it might, is an instability problem. Repeat dislocations, a new deformity after a fall, or a feeling that the ball is not staying in the socket belong with orthopedic care. A suspected full-thickness rotator cuff tear that needs a surgeon is another stop: a sudden rip while lifting, a new inability to hold the arm up, or weakness that is obvious even when the examiner takes pain out of the equation as much as a clinical test can. Those findings are referral findings. They are not a reason to spend a month "seeing if laser helps" while a repair window, if there is one, gets blurrier.

Fever with a hot, swollen shoulder, unexplained weight loss with deep night pain, chest pain or shortness of breath that you have been calling the shoulder, and new numbness or weakness that is spreading are medical emergencies or same-day medical problems, not adjustment problems. A fall in an older adult with pain at the upper arm bone needs imaging for fracture before anyone moves the joint through a heroic range. If you cannot tell whether the emergency is the shoulder or the heart, treat it as the heart until a medical team says otherwise.

Three patterns that share a sleeve and not a plan

Rotator cuff pain tends to live on the outer shoulder and worsen when the arm lifts, reaches, or carries. Lying on that side is a classic irritant. The cuff is a set of tendons that keep the ball centered while the larger muscles provide the power. Tendinopathy here is common. A partial tear and a full-thickness tear are not the same decision, and a webpage cannot tell them apart. What the exam can do is look at strength in specific directions, watch how the shoulder blade moves, and see whether the pain is tendon-dominant or whether the joint has simply stopped allowing motion.

Frozen shoulder, or adhesive capsulitis, is a stiffness disease as much as a pain disease. External rotation disappears early. The arm does not want to turn outward, and the loss shows up even when someone else moves the arm for you. That passive limit is the clue. A painful cuff can make you refuse to lift. A frozen shoulder stops the lift even when you are not the one doing the work. The history often runs in a sequence: a painful phase, then a phase in which pain eases and stiffness is the complaint, then a slow return of motion that is measured in months. Reaching a back pocket or fastening from behind becomes a specific, maddening failure. Diabetes and thyroid disease show up more often in this story than chance would suggest, which is a reason to mention them, not a reason for this office to manage those conditions.

Neck-referred pain is the pattern people least want, because the shoulder feels like the victim. If turning or extending the neck reproduces the shoulder ache, if the pain travels past the elbow, or if a nerve test in the arm is abnormal while the shoulder's passive motion is preserved, the neck is the project. Treating the deltoid with every tool in the cabinet will disappoint you. The neck exam, and what a chiropractor actually does with it, is described on what a chiropractor does for neck pain. A nerve-root pattern with weakness is its own guide: pinched nerve in the neck.

MedlinePlus lists shoulder injuries and disorders as a broad group, from strains to frozen shoulder to separation, which is why "shoulder pain" as a search is not a plan. Tendon overload has a separate overview on the tendinitis pages, and it behaves differently from a joint capsule that has tightened.

What the exam measures before anyone names a tool

Active motion is what you can do. Passive motion is what the clinician can do with your muscles relaxed. The gap between them is informative. A large gap, with good passive range and poor active strength, points toward tendon or muscle. A small gap, with both of you stopped at the same early angle, points toward a joint that will not allow the motion, which is the frozen-shoulder neighborhood. External rotation is checked on purpose, because that direction is the one adhesive capsulitis steals first. The shoulder blade gets watched, because a blade that wings or hikes changes the load on the cuff. The neck is screened even when you are sure the neck is innocent.

Palpation still matters. Tenderness at the outer tip of the shoulder is a different tissue from tenderness in the biceps groove or at the back of the joint. A plan aimed at the wrong centimeter is how people collect treatments without collecting function.

Range work, and the narrow role of machines

Frozen shoulder responds, when it responds, to time and to motion that respects the irritable phase. Aggressive stretching that leaves you flared for two days is not bravery. It is a dose error. Gentle, frequent motion into the limited directions, especially external rotation and reaching up and across, is the unglamorous center of care. Hands-on work to the stiff upper back can make the shoulder's job easier when the exam finds that the mid-back is part of the compensation. It does not dissolve a capsule by itself.

Laser or shockwave enters only if the exam says the pain is tendinous. A Storz radial or focused handpiece aimed at a tendon that is actually the source is a different decision from aiming it at a frozen capsule because the shoulder "hurts." The clinic's description of the devices is on the shockwave therapy page. Lateral elbow pain is a different tendon with its own page, shockwave for tennis elbow, and those details do not transfer to the cuff just because both regions are tendons. Class IV laser, including the 40-watt LightForce and the Summus in this office, is discussed on Class IV laser therapy. The Food and Drug Administration's laser guidance is blunt: a Class IV beam is an immediate skin and eye hazard in the direct or reflected path, and it may be a fire hazard. Eye protection and a trained operator are part of using it, not optional color.

What this office will not pretend to be

Chiropractic care here is joint assessment and treatment of the spine and extremities when the exam supports it, plus the physiotherapy and device options the practice actually owns. The chiropractic care and physiotherapy pages on the main site describe those services in the clinic's words. A comparison of chiropractor and physical therapist training, which this shoulder problem often needs side by side, is on chiropractor or physical therapist. The Bureau of Labor Statistics notes that chiropractors complete a doctoral-level professional degree and a state license, and that physical therapists complete a doctoral degree and a state license. Neither fact makes either person the surgeon you need if the shoulder is unstable or the cuff is fully torn and failing.

Dr. William Choi, DC, uses Diversified technique and came to chiropractic after a kinesiology degree. Dr. Matthew Richter, DC, is certified in Class IV laser and spinal decompression. Those credentials support the tools they use. They do not turn a chiropractic visit into an orthopedic shoulder clinic. If the story is instability or a tear that needs repair, the successful visit is a clear referral with your findings written so the next clinician does not start from zero.

Living with the months, without a cure slogan

Frozen shoulder is famous for outlasting everyone's patience. Improvement is often uneven: a week of easier dressing, then a flare after you painted a ceiling. Tracking one or two functions beats tracking a mood. Can you reach the pocket? Can you sleep on that side for part of the night? Can external rotation, measured the same way each time, move even a little? Those are the markers. A shot, when a medical clinician offers one, is their decision and their drug. This page does not recommend or forbid injections. It says they are not something a chiropractic office should casually imitate.

Cuff tendinopathy has a different marker: can you lift and carry with less night pain while strength in the weak direction returns? Loading the tendon, in a dose that does not flare you for days, is usually part of getting there. Complete rest feels kind and leaves the tendon just as unprepared for the next reach.

Bring a shirt you can move in, and be ready to show the exact reach that fails. Mention diabetes, thyroid disease, recent falls, and whether neck motion changes the pain. Ask, before you agree to a series, which of the three patterns the exam thinks you have. If the answer is "the shoulder, generally," the exam is not finished. A plan aimed at a tendon, a capsule, or a neck should be willing to say which one it is aiming at, and what finding would make the office send you to a surgeon instead.

Questions people ask

How can I tell a frozen shoulder from a rotator cuff problem?

Ask what happens when someone else moves the arm while you relax. Frozen shoulder, or adhesive capsulitis, stops the motion even then, and outward rotation is often the direction lost first. A cuff problem more often leaves passive motion available while active lifting is weak or painful, especially out to the side or overhead. Night pain from lying on that shoulder is common in cuff pain and does not settle the question. Reaching a back pocket can fail in either pattern.

Can a chiropractor help a frozen shoulder?

The help, when it fits, is mostly patient range work and care for stiff regions that are making the shoulder's job harder, such as the upper back. Frozen shoulder unfolds over months, and aggressive stretching that flares you for days is a dose error. A chiropractor can also notice when the story is not a frozen shoulder at all. Nothing here promises the motion will return on a calendar.

Could this shoulder pain actually be coming from my neck?

Yes. If turning or extending the neck reproduces the shoulder ache, if pain travels past the elbow, or if arm strength and reflexes are abnormal while the shoulder still moves passively, the neck is the project. Rubbing the deltoid will disappoint you. A shoulder that has lost passive motion is less likely to be only a neck problem. Both can exist. The exam should test the neck on purpose even when you are sure the neck is innocent.

When should a sore shoulder go to a surgeon instead of a clinic series?

Go sooner for a shoulder that has dislocated or feels as if it will, for a new deformity after a fall, and for a sudden rip with a new inability to hold the arm up. Weakness that remains obvious when pain is taken out of the test, as much as a clinical exam can do that, raises a full-thickness cuff tear that may need a surgical opinion.

Will laser or shockwave loosen a frozen shoulder?

Those devices are considered when the exam says the pain is coming from a tendon, not because the word shoulder appears on the schedule. A frozen capsule that has lost passive rotation is a motion problem. Aiming a Storz handpiece or a Class IV laser at it, without that tendon finding, is the wrong target. The office has both shockwave forms and two Class IV lasers, and the laser is an eye and skin hazard in the direct or reflected beam.

Waiting on the clinic

  • Medical sign-off. Dr. Matthew Richter, DC still needs to review and sign this page. Until that date is recorded, treat the page as a draft for clinical review, not as signed medical advice.
  • Doctor's own words. A short recorded statement from Dr. Richter or Dr. Choi belongs here. Do not publish a quote until one of them says it on the record.
  • Clinic photo still needed. The marked frame is waiting on a real photograph from the office. No stand-in patient or treatment photo will be generated.
